Know your population. Use data to understand what motivates your people. Our Transform reports can show enrollment, engagement, newly started, and milestone achievement. Leveraging this data can help you design for impact.

Regularly review your Transform reports, located in your Client Admin Portal’s Reporting Dashboards, with your Personify Health Account Team. Together, you can identify areas for improvement and ways to boost enrollment and engagement.

📩 Enrollment Campaigns: Spark Action Early 

The moment a member becomes eligible for Transform, we send a personalized email within 24 hours. Then, weekly reminders follow—up to four total—until they complete the survey and enroll. It’s not just a nudge. It’s a strategy to convert interest into action. 

🚀 Onboarding Campaigns: Set the Stage for Success 

Once a member schedules their first touchpoint, onboarding emails help them prepare. These messages clarify expectations, build confidence, and ensure they’re ready to start strong. 

🔄 Feedback Campaigns: Listen, Learn, Improve 

At week 9 and month 12, we ask members for feedback on their coaching and product experience. These insights help to refine the journey and keep engagement high.

  • Make sure you have these campaigns enabled for your members. One of the best marketing tools you have in your toolbox—email communications. These allow you to promote Transform to your members, support eligible members throughout Onboarding, and inform them of next steps and expectations. 

  • Looking for additional communications support? Ask your Personify Health Account Manager about the possibility of Custom Communications or setting up Webinars with Coaches to help drive enrollment and participation.

Nothing beats peer support. When a member shares their success, it does more than celebrate progress—it inspires action. Real stories from real people make health goals feel achievable and relatable, motivating others to stay engaged and stick with the Transform journey. It’s proof that small changes can lead to big results—and that success is contagious.

I got my blood test results back! Amazing news! My A1C is now 5.7! In April 2024, my A1C was at 6.1. After joining Transform in July, my October A1C was at 5.9. And it has now dropped to 5.7!

My doctor said to continue with exercise and diet modifications, and that all my other results (Lipid Panel, etc.) looked good. (Raw scores have all improved slightly. And my weight is about the same as last time we spoke: 120-122 range. I’ve also been getting in 10K steps about twice a week.).

Thanks a zillion, zillion, zillion for all your help! I think I have been thoroughly indoctrinated into Transform strategies, and as I’ve said: the proof is in the pudding! This success will motivate me to continue.

My favorite Transform strategies: the My Plate method; eating part of a cookie or dessert instead of the whole thing; eating my vegetables or salad first; dressing up any snacks with protein; counting chores like vacuuming or washing the car as exercise; parking far away to get in more steps; walking around the local shopping mall or medical center3-4x for extra exercise; and moving for at least 10 minutes after dinner or a meal. Like I said, success breeds success, so I will continue on with Transform. My new goal is to maintain at 5.7 and continue with 10Ksteps/a day.
