## From enrollment to empowerment: What members can expect 

Transform’s member experience begins with a short eligibility survey, designed to match members with the right program based on clinical guidelines and evidence-based recommendations. After they qualify for one of the programs, members receive a personalized recommendation and can self-enroll with just a few clicks.  

Once enrolled, members are paired with a certified health coach—many of whom hold advanced degrees and national board certifications. These coaches don’t just guide. They listen, motivate, and work with your member to create a care plan that fits their lifestyle, goals, and readiness to change.
